Co-pays are required at the time of service payable by credit/debit card, cash or check.
When visiting our offices, please bring your most current/active insurance card and a photo ID to ensure that your information is up to date and valid at the time of service.
Should your plan require any referrals, please be sure to obtain one before seeing one of our providers.
Please be sure to provide your employer name/phone number, date of injury, and workers’ compensation insurance carrier information (i.e. carrier, adjuster’s name, claim #, phone number). We will bill your commercial insurance should the workers’ compensation claim become denied/not accepted.
If the accident occurred in Connecticut and you have the medical provision (MedPay) on your auto policy, regardless if the accident was your fault or not, we must bill your auto carrier. Please provide the date of the accident, auto carrier’s name, address, adjuster’s name, claim # and phone number. We will bill your commercial insurance should there be no MedPay or if the MedPay has been exhausted.
If the patient was injured playing an organized school sport, be sure a claim form has been filed with your school’s sports injury accident insurance. This information will need to be provided to us along with your active commercial insurance card. We will bill the commercial insurance and school sports injury accident insurance accordingly.
